Summer Forms, Downloads, & More

This page includes information to help in preparing for the 2024-2025 academic year, including information about online permission and acknowledgment forms, packing for school, and FAQs. 

All families must complete ALL online forms by August 15, 2024.

ONLINE SUMMER FORMS

SUMMER FORMS WILL BE AVAILABLE VIA THE PARENT PORTAL MID-JULY

All permissions and acknowledgment forms are done through the secure parent portal, accessible when a parent is logged into Millbrook's website. Please note these steps to complete these important summer forms:
  1. Please login into the Millbrook website via the Login link at the top of the webpage. Click on the forms link in BLUE at the top of your landing page.
  2. All forms that need to be completed will be displayed. If you have more than one child at Millbrook, you will need to complete a form for each child.
  3. There are forms that need to be signed by at least one parent and the student including the Computer Use and Library Use forms. Although two parent signature boxes might be displayed, only one parent must sign. The student must log in with his/her own username and password in order to sign these forms and complete the submission process.
  4. In addition, all parents should review the contact information we have on file. It is important that we have correct address, phone, and email information for each parent. When logged in, click on the down arrow next to your name in the top navigation bar, then click "Profile." Please review your profile and make any changes, if necessary, by clicking on the pencil icon next to the information you need to edit.
  5. Parents should also download important information on the right side of this screen. This includes general lists of what to bring for boarding students, computer specifications, and more. All of these downloads will be available here and on your Parent Resource Board throughout the year.

Following is the required summer reading for students who will be attending Millbrook for the 2024-2025 school year; this list should be read according to the form students will be entering for the upcoming school year. This reading is assessed at the very beginning of the school year, and so it is important that your child completes it.

Please make arrangements to purchase these books at your local bookstore or via an online bookstore of your choosing.

    From the Parent Resource Board, click the Magnus Health Portal button to provide and update all health and immunization records.

    • How do I retrieve my belongings if I used the moving service that is partnered with Millbrook School last year (applies to returning students only)?

      If you used the moving service that is partnered with Millbrook School, your belongings will be waiting for you at your dorm building upon your arrival to campus.
    • When are student arrivals for the start of school?

      We will have staggered arrivals for the start of the 2023-2024 school year:

      Wednesday, September 4: student leaders, pre-season athletes and international students arrive 

      Friday, September 6
      : returning students and international students arrive

      Saturday, September 7: new students and international students arrive

      On Sunday, September 8, all students will be on campus, and we will follow a special schedule. Classes begin on Monday, September 9.

      We will be in touch in August about specific arrival times on each day.
    • Are there general packing guidelines that I should be aware of?

      Lamps, stereos, clocks, radios, computers, fans, and hair dryers are permitted.

      The following electrical appliances are not permitted in student dorm rooms:
      • refrigerators
      • toasters
      • hot plates
      • heating coils
      • hot-pots
      • air conditioners
      • sun lamps
      • televisions
      • electric blankets
      • microwave ovens
      • space heaters
      • gaming systems
    • How do/when can I ship items to Millbrook School for the upcoming year?

      Packages may be shipped to the school throughout the last week in August. They should be addressed to the student (please see example below):

    • How does laundry work?

      The school has washers and dryers available for student use in each dorm, and students may purchase laundry cards in the School Store. These heavy-duty machines can handle sheets and general laundry.

      In addition, the school has made arrangements with E&R Laundry to provide a personal laundry service for students. E&R laundry will send a flyer out to all boarding students in the summer regarding how to sign up. 
    • When do I find out my dorm assignment?

      You will get your dorm assignment when you arrive on campus to register for the start of school.
    • Where can I find information about ordering course books and the online book store?

    • How do I access the Magnus Health forms?

      Magnus Health can be accessed via the Parent Resources Board on the Millbrook School website. After logging into the Millbrook website via the "Login" link at the top right portion of the Millbrook homepage, you will be directed to the Parent Resource Board and see an icon that links directly to the Magnus Health Portal. No further username or password is required.

    • How can I submit my forms to Magnus Health?

      You will complete most Magnus forms online. Only three forms (immunization, proof of insurance, physical exam) must be downloaded, printed, completed by you or your doctor, and then submitted back to Magnus.

      There are three options to submit the three documents that must be printed:
      1. fax them to Magnus
      2. scan and then upload them to the Magnus Health mobile app for Android and Apple devices
      3. scan and then upload them to the Magnus Health portal from your computer
      Contact information for mailing and faxing is printed for your reference on each form.

      Please note that a cover sheet prints out with each of the three documents that must be completed by your doctor. This cover sheet must accompany each document that you return to Magnus.

      Please DO NOT SEND ANY HEALTH FORMS TO MILLBROOK SCHOOL.

    • Do you store Magnus Health documents in hard copy anywhere at Millbrook School?

      No, we do not store these forms and documents in the health center or anywhere at else at Millbrook School. They are found only on the secure Magnus Health portal.
    • How can I contact Magnus Health directly?

      You can reach Magnus Health directly by phone at 877-461-6831 or via email: service@magnushealthportal.com.
